Admission

To obtain services from Mackay Rehabilitation Centre, clients must have received a diagnosis by a health care professional in one of the following areas:

• Motor impairment: aged between 0-18 years
  (18-21 years old that are currently attending a high school)



• Speech or language impairment : aged between 0-18 years
  (18-21 years old that are currently attending a high school)



• Deaf or Hard of Hearing: all ages
  

The referral source must attach reports documenting the diagnosis along with all other pertinent medical or professional reports (OT, PT, Psychology..) to the Request for Services Form (DSIE form for professionals working within the Ministry of Health and Social Services).
